4RocksSucess Physical Therapy strives to provide exceptional patient-focused care, determined by your physical therapist and not limited by insurance plan.  We believe in the value of the specialized one-on-one services that we provide.  As a courtesy, Success Physical Therapy will provide education regarding filling out forms needed to submit claims to your insurance company for reimbursement of services.

Insurance and Payment Information

  • Success Physical Therapy has chosen to be a fee-for-service based practice and operates as an out-of- network provider of physical therapy services.  

  • We have chosen not to establish contracts with any insurance companies because we believe in providing physical therapy services according to individual need, rather than based on insurance benefits.

  • We do not have to limit the time or quality of treatment provided because of insurance company restrictions, or elevate our rates to pay for billing services.

  • Additionally, we have the flexibility to assist patients in financial need.

  • Prior to your first scheduled appointment, call your insurance company to completely understand your physical therapy benefits.

Insurance Benefits Worksheet

  • At the time of service and payment, you will receive a receipt, which you can submit to your insurance company for their consideration of reimbursement to you.  We will be happy to provide chart notes or other documentation at your or your insurance company’s request.

  • The amount of reimbursement you receive will vary according to the terms of your insurance policy.  Some companies may reimburse you at 80%, some at 60%, some at 40%, and some may not reimburse you at all.  We cannot make guarantees or estimates regarding what reimbursement your plan allows.

Payment and Fees

  • We accept Cash, Check or Visa/MasterCard at the time of service.

  • Fees are based on time spent and the treatments performed during your appointment.

  • Payments are due in full at time of service.

Cancellation and No Show Policy

  • We reserve 60 – 90 minutes for each appointment and plan your treatment sessions accordingly to achieve your goals.

  • Frequent cancellations or no-shows may delay your progress and prolong your symptoms.

  • We will make every effort to remind you of appointments in a timely fashion to allow you sufficient time to make arrangements if you need to cancel, however it is ultimately your responsibility to keep track of your appointment schedule.

  • A missed appointment or cancellations with less than 24 hour notice will be subject to $75.00 late fee.

  • The cost of your last visit will be charged for additional late cancellations or no shows.

  • Late arrivals to appointments will be charged as a full appointment of the scheduled service.
